Important Information About Your Office Visit
What to Bring
If you are seeing or have seen a psychiatrist or therapist, please bring their information for the consent to release form. Please also bring prescription information for all medications that you may be currently taking.
Insurance Information
Dr. Rosenbaum does not take insurance directly, but she will document on your invoice the information needed for you to submit to your insurance carrier for reimbursement. Please find out from your insurance carrier what your deductible is and what they will reimburse you for each visit.

If you need to reschedule your appointment or have any questions, please contact the office directly at 917-748-5428. For new consultations, please give 24 hours advance notice if you are unable to make your scheduled appointment. More advance notice may be required for follow-up appointments.
